Your confidential information will be collected, shared and stored in many different ways. Information about you will be collected directly from you or your carers and from 3rd parties such as healthcare professionals, family members and friends or your representatives. Your information will be shared and stored through, recorded conversations either face to face or over the phone, in paper records, in digital files held on computers or cloud services, e-mails, voicemails, electronic messaging, video (CCTV) records, images or photographs

You should raise concerns if you feel your confidentiality has been breached or your information is not be used in the correct way or as you intended

If you choose not to share certain information, it might limit our ability to provide you with the best possible care. In some cases, we may not be able to proceed with your care or offer certain services

We will keep your confidential information for as long as necessary to provide your care and as required by law. This includes information collected when we are agreeing to care for you, the updates we make when care is being provided and for a period, after we stop providing care. After that, we will securely delete or anonymize your information

• Strict Privacy Standards: We follow strict guidelines to ensure your information is kept private. Only those who need to know your information, such as your care workers and healthcare providers, have access to it

• Secure Storage: Your details are stored safely, whether on paper or in electronic form. We use secure systems to protect your information from being accessed by anyone who shouldn’t see it

• Limited Sharing: We only share your information with those involved in your care, like doctors or pharmacists. We will always ask for your permission before sharing your details with others, unless it’s an emergency

Your information may be shared with trusted partners, for example our care management platform provider, specialists or labs, who help us provide your care. These partners are required to keep your information safe and use it only for your healthcare needs

There are certain situations where we may need to share your information without your explicit consent, but only if it is in your best interests. 

This may include:

• Emergency Situations: If you are unable to provide consent due to a medical emergency, we may need to share your health information with medical professionals to ensure you receive the appropriate care

• Protecting Your Well-being: If sharing your information is necessary to protect your health and safety, such as in cases where you may be at risk of harm, we will act in your best interests

• Legal Obligations: In some cases, we may be required by law to share your information with authorities, such as safeguarding agencies or law enforcement, if it is necessary to protect you or others

In all cases, we will ensure that your information is only shared with those who need to know it and that it is handled with the utmost care and confidentiality

• Making a Formal Complaint: If you believe that your information has been mishandled or your rights regarding your information have not been respected, you can make a complaint with the Information Commissioner’s Office. They can help address your concerns

In the unlikely event that your confidentiality is breached, we will:

• Investigate Immediately: We will look into what happened and how it occurred

• Inform You Promptly: We will let you know what has happened and how we are addressing it

• Take Corrective Action: We will put measures in place to prevent it from happening again

• Support You: We will provide support and assistance to help you feel reassured

You have the right to

• Request access to the information we hold about you

• Ask us to correct any information that is not correct

• Ask us to delete your personal information if it is no longer needed for the purposes we collected it

• Know how your information is being used and restrict how its used

• Ask us to transfer your information 

• Tell us not to use your information. You can withdraw consent at any time and it will not affect any care you’ve already received

We will confirm receipt of your request within five days. We expect to provide you with a full reply within 30 days. If we cannot do that we will contact you and provide an explanation and give you a new expected timeframe
