Information about you will be collected directly from you or your carers and from 3rd parties such as healthcare professionals, family members and friends or your representatives. Your information will be shared and stored through, recorded conversations either face to face or over the phone, in paper records, in digital files held on computers or cloud services, e-mails, voicemails, electronic messaging, video (CCTV) records, images or photographs
You should raise concerns if you feel your confidentiality has been breached or your information is not be used in the correct way or as you intended

This includes information collected when we are agreeing to care for you, the updates we make when care is being provided and for a period, after we stop providing care. After that, we will securely delete or anonymize your information

We will confirm receipt of your request within five days. We expect to provide you with a full reply within 30 days. If we cannot do that we will contact you and provide an explanation and give you a new expected timeframe
